You must perform a dodge and then three separate/unique throw escapes. You must do this and successfully dodge an attack, or successfully escape a throw. In other words, if you try this when the opponent isn't attacking or throwing you, then it won't count.

In practice, after one of your moves is guarded, enter the following very quickly:

~ + ~ + ~ +

If the opponent tries to attack you, you'll dodge with the input. Or if they tried to throw, you'll escape if the throw matches one of your throw escape inputs.
